Insurance & Payment Policy (Self-Pay Practice)
ONE Vida is a self-pay medical practice and does not accept insurance, including private insurance, Medicare, or Medicaid.
ONE Vida does not bill insurance or contact insurance carriers on a patient’s behalf. Upon request, we may provide a superbill containing standard information typically required by insurance companies for reimbursement. Submission and reimbursement are the patient’s responsibility, and reimbursement is not guaranteed.
Payment in full is due at the time of service. If payment is not made in full, additional fees may apply, and services may be delayed or declined.
Laboratory/Blood Work Services
While ONE Vida does not accept insurance, a copy of the patient’s insurance card is required when laboratory testing is ordered so that specimens may be sent to an external laboratory. The laboratory will bill the insurance company directly for lab services.
Any charges, denials, or reimbursement related to laboratory services are strictly between the patient, the laboratory, and the insurance company. ONE Vida is not responsible for laboratory charges, insurance reimbursement, or disputes related to lab billing.
Credit Card on File
Authorization & Financial Policy Acknowledgment:
To help make billing easier and more convenient, ONE Vida requires a credit card to be kept on file.
All patients must authorize ONE Vida to securely store credit card information and charge it for any balances that remain unpaid. This may include, but is not limited to:

Any charges associated with visit services

Any balance remaining if services are paid with an alternate form of payment

Any late-fees, cancellation, or no-show fees
